Two common causes. Devices with a barometric altimeter drift when air pressure changes during a ride, which can shift the whole profile by tens of metres. And many exports and format conversions simply drop the elevation tag, leaving a completely flat profile.

No. The GPX is parsed and rewritten in your browser. Only a list of coordinates is sent to the elevation service to look up ground height, and nothing is stored.
Not exactly, and that is expected. Terrain data gives the height of the ground, so bridges, tunnels and multi-storey car parks are reported as the surface beneath them. For ordinary road and trail routes it is usually closer to reality than a drifting barometer.
Elevation is sampled along the route rather than at every single track point, then interpolated between samples. That removes the metre-by-metre noise you get from a GPS recording while keeping every real climb and descent.
